Health Requirements and Forms
Health Requirements for Registration:
- PreK- Physical/ IDPH Immunizations
- Kindergarten- Physical/IDPH Immunizations, Eye Exam and Dental Exam
- 2nd grade- Dental Exam
- 6th grade- Physical/IDPH Immunizations, Dental Exam
- 9th grade- Physical/IDPH Immunizations
- 12th grade- MCV4 Immunization after 16th birthday
- Students new to Illinois Schools- Physical Exam (Illinois form or equivalent), IDPH Immunizations, and Eye exam (Illinois form or equivalent), all due within 30 days of enrollment.
Dental Exams due by May 1
Physical Exam/Immunizations or appointments cards due at registration
Eye Exam or appointment cards due at registration
